Scope of Services
- Nervana Health provides health coaching and educational services designed to support lifestyle, wellness, and personal growth.
- Nervana Health does not provide physical therapy, medical diagnosis, or medical treatment through coaching sessions.
- Although I am a licensed physical therapist, I am not acting as a physical therapist when providing coaching services. Physical therapy is only provided separately, under Utah licensure, and is not part of Nervana Health.
Not Medical Advice
- Coaching is not a substitute for medical advice, diagnosis, or treatment.
- Always consult with your qualified healthcare provider regarding any medical condition, medication, or treatment decisions.
- Never disregard or delay medical advice because of something discussed in a coaching session.
Licensing & Jurisdiction
- I am licensed as a physical therapist in the state of Utah.
- Coaching services provided through Nervana Health are not physical therapy and are not subject to PT licensing rules or regulations.
- Nervana Health does not claim licensure or certification in any state for coaching.
Privacy & Confidentiality
- Sessions are conducted virtually. I may take personal notes for tracking and reference.
- Nervana Health does not use an electronic medical record (EMR) and is not HIPAA-compliant.
- While reasonable efforts are made to protect your privacy, absolute confidentiality cannot be guaranteed.
Client Responsibilities
By booking services, you agree that:
- You are voluntarily participating in coaching.
- You remain responsible for your own medical care and health decisions.
- You will consult your healthcare provider before making significant health or lifestyle changes.
Eligibility
- You must be at least 18 years old to book services.
- If under 18, a parent or guardian must provide written consent.
Technology Disclaimer
- Coaching sessions are provided virtually. Nervana Health is not responsible for technical failures (e.g., internet, software, or device issues).
- Clients are responsible for having the necessary equipment and internet connection.
Intellectual Property
- Materials provided during or after sessions (including worksheets, handouts, and recordings) are for personal use only.
- Materials may not be copied, shared, or distributed without written permission.
Payment, Cancellations & Refunds
- Payment is due at the time of booking unless otherwise agreed.
- Sessions cancelled with less than 24 hours’ notice may be charged in full.
- Refunds, if any, are at the discretion of Nervana Health.
Termination of Services
- Nervana Health reserves the right to refuse or discontinue services at any time, including cases of inappropriate behavior, unsafe conduct, or misuse of services.
Emergency Disclaimer
- Nervana Health does not provide emergency or crisis services.
- If you are experiencing a medical or mental health emergency, call 911 (or your local emergency number) or go to the nearest emergency department.
No Guarantees
- Results vary from person to person.
- Nervana Health makes no guarantees or warranties regarding specific outcomes or improvements.
Limitation of Liability
- To the fullest extent permitted by law, Nervana Health is not liable for any injury, loss, or damages arising from participation in coaching services.
Indemnification
- By using these services, you agree to indemnify and hold harmless Nervana Health and its representatives from any claims, damages, or liabilities related to your participation.
Governing Law
These terms are governed by the laws of the United States and the state in which Nervana Health operates.
Severability
If any portion of these Terms is found unenforceable, the remaining sections remain in effect. These Terms represent the entire agreement between you and Nervana Health.
Agreement
By scheduling or attending a session, you confirm that you have read, understood, and agreed to these Terms of Service.
